58. SNARE

SNARE stands for SNAP receptor, SNAP stands for soluble NSF attachment protein, and NSF stands for N-ethylmaleimide sensitive factorMany neurotoxins directly affect SNARE complexes. Such toxins as the botulinum and tetanus toxins work by targeting the SNARE components. These toxins prevent proper vesicle recycling and result in poor muscle control, spasms, paralysis, and even death.View image by clicking hereNote:

Clinical Type Question (CTQ)

In a cricket match, the fast bowler injured his shoulder while fielding at the boundary. He could not bowl any further in the match and was sent to the orthopedician for further evaluation  His emergency MRI showed a rotator cuff injury. All of the following tendons could be affected in this injury except ? A.   Supraspinatus B.    Subscapularis C.   Teres Major D.   Infraspinatus

57. Wolman Disease

Autosomal recessive.Lysosomal Acid Lipase deficiencyPresents with failure to thrive in the first few weeks after birth, relentless vomiting, abdominal distension (hepatosplenomegaly due to accumulation of cholesteryl esters), steatorrhea and death usually occurs within 6 months.Similar  accumulation of cholesterol and fatty acid crystals also occurs within the cells of the adrenal cortex, resulting in enlargement of the adrenal glands - leading to bilateral calcification of adrenal glands - pathognomonic.

15 Image Minutes

I am sure you guys are looking at all the images in your Apps, tests, videos, Qbanks etc.... and ALSO the images being posted by your faculty and mentors. These are definitely useful...YES. BUT, Along with these - Spend 15 minutes/day going through images from your textbooks (yes those fat things made of paper that you used to study during your MBBS course !!). Especially books like Surgery textbook; Medicine textbook; OBG textbook; Pathology textbook, Ortho and Peds and Ophthal and ENT textbooks. Usually these are standard images and then read the text below the images also. This will instil confidence in you and by April 18th you would have gone through hundreds of "potential exam images". Give yourself these 15 images minutes daily. You'll thank me later All the Best Dr Ramgopal

Strategy, Tips Etc...

Lengthy is NOT always difficult !

The examination shall be a multiple choice questions delivered using computers network (CBT) as per scheme prescribed. The exam comprises of 200 Multiple Choices, single correct response questions in English language only. Time allotted is 3 hrs 30 min.  The above information in the NEET PG bulletin released today is creating much confusion among students. However - Points to note  200 MCQs in 210 minutes - 57 seconds (almost 1 minute) per question (earlier you had just 42 seconds) Single Correct Response - Normal MCQ type (Not INICET type)The medical knowledge does NOT change AND What you have already studied is NOT a wasteEven if question stem is long - time available is longer to arrive at the answer; ALSO number of questions are lesser (200 compared to 300). Hence "shortage of time" is unlikely to occur. Many times - overthinking complicates things -  Keep it simple - LONGER questions usually have enough clues within the question to arrive at the answer I wouldn't change my preparation at this point - rather I would feel a little more comfortable with the additional time available ( lesser questions) - since I also know that NEET PG 2020 had 300 clinical questions in 210 minutes (so this 2021 NEET PG will definitely be doable)  Could all 200 be clinical type questions ?? - YES - BUT  they haven't mentioned that clearly - hence I do expect a few (at least 10-15%) one-liners to creep in here and there).  And as we know, even the juicy full toss has to be hit to the  boundary - meaning that these one liners also have to be got correct along with other lengthier questions. Ophthalmology

56. ISNT rule

In a healthy patient, in the optic nerve, the neuroretinal rim typically is thickest in the inferior (I) area, followed by the superior (S), nasal (N) and temporal (T) areas = ISNT Rule.(Thickest I >S>N>T thinest)However, in a patient with glaucoma, the cup often is elongated vertically. This translates into thinning of the inferior and superior rim tissue, which ultimately violates the ISNT rule. Marked deviation from the ISNT rule is a potential indicator of a glaucomatous process.

Surgery

55. MELD Score

Model for End-Stage Liver Disease (MELD) scoreMELD is a useful tool in predicting the mortality from liver disease (and thus, how urgently an individual needs a liver transplant) in the next three months.  The MELD score is based on a patient’s serum Creatinine, Bilirubin and INR (CBI) ! The score ranges in value from 6 (lowest priority) to 40 (highest priority).MELD-Na score (an extension of MELD) includes serum sodium - this may help to better rank candidates.

Microbiology

54. Pilot wheel appearance

Paracoccidioides are dimorphic fungi. In its mycelial form, Paracoccidioides species appear as thin septated hyphae with occasional chlamydospores and conidia. In the yeast form, Paracoccidioides species are characterized by oval or round budding yeast cells of varying sizes (4 to 40 microns). The typical appearance is that of a large mother cell surrounded by multiple budding daughter cells (blastoconidia), often called a "pilot's wheel" . When the mother cell has only two budding cells, it may resemble a "Mickey mouse head."
